When most people hear the word "chemotherapy," they picture something intense — big doses, hard side effects, a dog who feels miserable through treatment. Chlorambucil often tells a different story. It's one of the milder, more flexible chemotherapy drugs in veterinary oncology, frequently given at home in low daily or every-other-day doses, with the goal of keeping cancer quiet and your dog comfortable for as long as possible.
If your veterinarian or oncologist has mentioned chlorambucil for your dog, here's a clear, grounded look at what it is, how it works, what to expect, and the safety steps that matter most.
What Chlorambucil Is
Chlorambucil is a chemotherapy drug in a family called the nitrogen mustard alkylating agents. It's actually a human medication — sold under the brand name Leukeran® as a 2 mg tablet — that veterinarians use "off-label," meaning it's prescribed for dogs based on clinical experience and research even though it wasn't originally developed for them. That's a common and accepted practice in veterinary oncology.
Compared with some other alkylating agents, chlorambucil is generally considered to have a gentler side-effect profile, which is part of why it's so useful. It also has a second role beyond cancer: it can act as an immune-suppressing drug for certain immune-mediated diseases, a use that's actually more common in cats than dogs. In the body, chlorambucil is processed by the liver and cleared through the urine. It usually comes as a tablet, but a veterinary compounding pharmacy can prepare it as a capsule or liquid if your dog tolerates those forms better.
How Chlorambucil Works
To understand chlorambucil, it helps to know that cancer is, at its core, a problem of cells dividing when they shouldn't. To divide, a cell has to unwind and copy its DNA. Chlorambucil interferes with exactly that step.
At higher doses, the drug cross-links the building blocks of DNA — essentially knotting the strands together so the cell can't unwind and copy its genetic material. Without that ability, the cell can't divide and produce more cancer cells. Importantly, chlorambucil doesn't single out cancer cells specifically; it acts on any rapidly dividing cell. That's why side effects can show up in the body's other fast-dividing tissues, like the lining of the gut and the bone marrow.
There's a second, quieter way chlorambucil can work, and it becomes important at low doses. This is the idea behind metronomic chemotherapy — giving a small amount of the drug continuously over a long stretch of time, rather than large doses spaced a week or more apart. Given this way, chlorambucil is thought to work less by attacking the tumor directly and more by interfering with the tumor's blood supply, choking off the new blood vessels a tumor needs to grow. Because the doses are low and steady, this approach is often easier on the dog and tends to cause fewer side effects.
What Chlorambucil Is Used For
Chlorambucil's history in veterinary medicine began at higher doses for cancers of the blood and immune system, particularly various forms of lymphoma and leukemia. In these settings it's often paired with the steroid prednisone, and it can be one component of a multi-drug protocol. It can also stand in for cyclophosphamide — another alkylating agent — in combination protocols when a dog doesn't tolerate cyclophosphamide well.
A few uses are especially worth knowing about:
- Low-grade gastrointestinal lymphoma. Chlorambucil has shown real promise for this slower, more indolent form of lymphoma that affects the gut. It's an uncommon diagnosis, making up only about 5–7% of canine lymphomas, but it's one where chlorambucil-based treatment has been a meaningful option.
- Mast cell tumors. When combined with a steroid, chlorambucil has been studied as a treatment for mast cell tumors that can't be removed surgically.
- A range of other tumors, particularly in the metronomic, disease-control setting described below.
Metronomic Chlorambucil: A Different Goal
Here's a mindset shift that's important for families to understand. With metronomic, low-dose chlorambucil, the goal usually isn't to eliminate the cancer. The goal is to stop it from progressing — to hold the line and preserve quality of life. Some tumors do shrink or even go into remission on this approach, but stability is the realistic and worthy aim.
The research paints a measured but encouraging picture. In one study of 36 dogs with a variety of tumor types — half of whom already had cancer that had spread when the study began — dogs received chlorambucil at roughly 4 mg/m². A small number achieved complete remission (including dogs with a mast cell tumor, a soft tissue sarcoma, and a thyroid carcinoma), but the more striking finding was that nearly half the dogs had stable disease, meaning their cancer didn't get worse. Median survival in that group was around 135 days.
Later work that looked at a range of doses, up to 8 mg/m², concluded that about 4 mg/m² was the sweet spot — effective while keeping significant side effects to a minimum. And in a study of 21 dogs with mast cell tumors treated with chlorambucil and prednisolone, roughly 43% showed some degree of remission and about 37% had disease that held steady, with a median survival around 143 days. The authors of that work noted that survival times were comparable to other protocols, but with the potential advantage of fewer side effects.
The throughline across these studies: chlorambucil rarely produces dramatic, total-cure results, but it can buy good time — weeks, months, and sometimes longer — often while keeping a dog feeling well. How any individual dog responds depends on the specific cancer, her overall health, and how she responds to her whole treatment plan.
Side Effects and Safety
For most dogs, especially on low-dose metronomic protocols, chlorambucil is well tolerated and side effects are minimal. Some breeds, such as Poodles and Kerry Blue Terriers, may experience hair loss. Dalmatians taking allopurinol may need a dose adjustment.
The more serious side effects to watch for relate mostly to the gut and the bone marrow. Contact your veterinarian promptly if your dog shows any of the following:
- Lethargy or unusual tiredness
- Vomiting or diarrhea — and especially bloody vomit or stool — or loss of appetite
- Skin bruising or unexplained bleeding
- Difficulty breathing
- Muscle or eye twitching
- Anxiety or restlessness
- Seizures
Because chlorambucil suppresses rapidly dividing cells, your veterinary team will typically monitor your dog's bloodwork during treatment to keep an eye on red cells, white cells, and platelets.
A Safety Issue That's Easy to Overlook: Protecting Yourself
This part deserves real attention. Chemotherapy drugs don't just affect the patient — they can affect the people handling them. Chlorambucil can be absorbed through your skin, and repeated exposure can be harmful over time, particularly for anyone who is pregnant or immunocompromised.
If you're giving chlorambucil at home (as is common with metronomic protocols), follow these precautions every time:
- Wear gloves to handle the medication and to clean up after your dog.
- Wash your hands before and after putting on and removing gloves.
- Dispose of gloves and your dog's waste in the trash, sealed, rather than handling them bare-handed.
- Return leftover medication through a drug take-back or recycling program rather than throwing it loose in the trash or flushing it.
- If you are pregnant, do not handle the drug or your dog's waste at all — ask another household member to take over these tasks.
- Don't let your dog urinate or defecate in public areas while on treatment, to protect other people and animals from exposure to drug residue.
Chlorambucil should be stored in the refrigerator and protected from light.
How It's Given, and What to Do About a Missed Dose
Chlorambucil is given by mouth, and depending on the cancer and protocol your veterinarian chooses, the schedule may be daily, every other day, or every one to two weeks. It's generally given on an empty stomach, because giving it with food reduces how well it's absorbed.
If you miss a dose, give it as soon as you remember — unless it's nearly time for the next scheduled dose, in which case skip the missed one and return to the regular schedule. Never double up to make up for a missed dose. Giving extra chemotherapy at once can be harmful. When in doubt, call your veterinarian for guidance specific to your dog's protocol.
When Chlorambucil Isn't the Right Choice
Because chlorambucil suppresses the immune system, it generally shouldn't be used in dogs who are already immunosuppressed for some reason other than their cancer, or who have an active infection. Like other chemotherapy drugs, it should not be given to pregnant animals.
Drug interactions matter here too. Chlorambucil has noted interactions with immunosuppressive drugs, certain vaccines, and a long list of other veterinary medications. Even the herbal supplement echinacea is worth limiting, since it may reduce chlorambucil's effectiveness. This is exactly why your veterinarian needs a complete, up-to-date list of every medication and supplement your dog takes — including the "natural" ones — before adding chlorambucil to the plan.
When to Call Your Veterinarian Right Away
Reach out promptly if your dog develops marked lethargy, vomiting or diarrhea (especially with blood), loss of appetite, bruising or unexplained bleeding, difficulty breathing, muscle or eye twitching, unusual anxiety, or seizures — or if you accidentally give too much or have a question about a missed dose.
The Bottom Line
Chlorambucil won't be the right fit for every dog or every cancer, but it fills a valuable niche: a relatively gentle, flexible, often at-home chemotherapy that aims to keep cancer in check while protecting your dog's quality of life. For the right patient — a dog with low-grade gastrointestinal lymphoma, an inoperable mast cell tumor, or a cancer where steady control is the goal — it can offer meaningful, comfortable time together.
As with any chemotherapy, the details matter: the dose, the schedule, the monitoring, and the handling precautions are all part of using it safely. Work closely with your veterinarian or a veterinary oncologist to decide whether chlorambucil belongs in your dog's plan, and lean on them for the specifics tailored to your dog's diagnosis and overall health.
